Pediatric Surgery: An Overview & Treatments Available in Udaipur

Pediatric surgery is a special form of medical treatment related to surgeries among infants, children, and adolescents. In the 20th century, pediatric surgery surfaced because the surgical procedure of birth defects needed noble techniques. Pediatric surgery is a separate specialty. The surgeons and the doctors need to be extremely careful while dealing with their patients. Their patients are not full-grown adults who understand the procedure and the pain they might need to go through but are children who have different physiology and psychology.

Pediatric surgery is a complex process and phase for the child but more than that for the parents. Seeing your children go through the suffering and letting them undergo complex medical procedures is a huge challenge in itself.

Pediatric operations have different effects on different children; the child needs better counseling from the parents and the doctor before getting into any kind of procedure. There are many kinds of pediatric surgeries which are generally related to urological, gynecological, cardiothoracic, and general issues.

Before proceeding further with the surgical procedure, the child needs to be appropriately examined for any other pre-existing conditions or ailments that can cause trouble or complications. An excellent pediatrician for children is able to detect and solve these problems at ease.

The various categories in which pediatric surgeries have been divided are described below:

What are the different types of pediatric surgeries?

Pediatric surgeries are divided into four categories. The doctor needs to understand better the need and perfect time for the surgery of their child. The four categories are:

  • Major surgeries: The surgeries related to head, neck, chest, and some abdominal operations are called major surgeries. These surgeries have complex procedures and need to be planned properly. These surgeries have a more extended recovery period and have longer hospitalization time. Pediatric operations like tumor removal from the central nervous system, congenital heart defects are included in this category. Other procedures like correction of skull malformations, gastrointestinal surgeries, etc., are included in this.
  • Minor Surgeries: Minor pediatric surgeries are the one which includes less complicated procedures and have shorter recovery times. The Minor pediatric surgical procedures include removal of excess skin, biopsies, and correction of long bone fractures. The chances of complications in these types of surgeries are close to zero only. The children are generally allowed to go home the same day or at a comparatively lesser time.
  • Elective surgeries: Elective surgeries are the types of surgeries that can be avoided for an extended period of time. These are the ones where the child does not necessarily need to be operated on. The surgeon can postpone the dates according to convenience, for example, the surgical procedure of circumcision in boys.
  • Urgent Surgeries: These are Surgeries that need to be performed immediately, without which the child won’t be able to survive. These include fatal injuries caused during accidents or congenital heart diseases at the time of birth. A minor delay in these procedures can cost a life.

Now that we know about the different types of pediatric surgeries let us know about the 10 most common pediatric surgeries.

What are the most common Pediatric Surgical Conditions?

These are the 10 most commonly performed pediatric operations:

  1. Cancer treatments: Various types of cancers related to the liver, lungs, and other organs and parts of the body can nowadays be found among children.
  2. Appendectomy: When the appendix becomes infected or painful, it needs to be removed.
  3. Gallbladder surgery: The Gallbladder can get infected due to a stone or a virus, this can be extremely painful for the child, and surgery for the removal needs to be performed.
  4. Bowed Chest: This condition occurs when the breast bone and the ribs of the child grow disproportionately.
  5. Biliary Atresia: This is a defect caused during the time of birth where the bile duct fails to empty properly.
  6. Congenital diseases: Also called birth defects, congenital disorders, develop prenatally and may be identified before or at birth, or later in life. Many organs can be deformed due to these diseases. The surgeon needs to reconstruct such organs and prescribe pediatric medicine for a quicker recovery of the child.
  7. Bariatric Surgery: A treatment for childhood obesity which includes the insertion of a balloon inside the oesophagus to reduce the appetite.
  8. Diseased Intestines: At the time of birth, the cells of the child’s intestines can be severely damaged and this might need the surgical treatment of removal of diseased intestines.
  9. Reproductive defects: The child can suffer from congenital disabilities in the reproductive organs, and a surgical procedure might be needed as a solution.
  10. Imperforate Anus: Many children are born with the anal opening being at the wrong place. This needs to be corrected with the help of surgery to allow the waste material to pass through the bowel.

We know that it is a difficult phase for a child when undergoing pediatric surgeries, and the child can be frightened of this. The child needs to be consoled properly before any procedure. Here is a better guide on how to explain pediatric surgery to your child.

How to prepare your child before pediatric surgery?

Pediatrics in udaipur
Before the surgery, the child can have many questions and queries regarding the surgery. It would be better if you don’t hide anything from the child as everyone likes to get the right and true answers. Before the surgery, the parents can tell the child about all the required information about the surgery.

The child may also be allowed to have a few or more than a few words with the pediatric surgeon or the pediatrician before the surgery as this will help them to better trust the surgeon and calm themselves.

These were some methods that can help you better deal with your child before a pediatric surgery.

If one weighs at least 20% more than your standard weight, you could be considered obese. In other words, if the body mass index is 30 and above, then it is considered obese. Obesity is a growing concern in various parts of the world. Having excessive body fat, due to hereditary or dietary choices is considered obesity. It can affect anybody, there is no specific age group that is more prone to obesity.

Obesity leads to various other diseases like heart diseases, high blood pressure, diabetes, and certain types of cancer. Obesity is not merely a cosmetic abnormality but can be fatal for the body if not controlled at the proper time. Obesity is vastly known to be a cause of genetics or due to an unhealthy lifestyle. It causes other diseases caused by sedentary life like fatigue and lethargy. There are various procedures in General medicine and Surgery that help medically remove excess fat.

How does obesity contribute to Diabetes?

diabetes and obesity

Excess body weight, an escalating problem worldwide, is a major risk factor for cardiovascular diseases and Diabetes.
Diabetes affects more than 180 million people around the world, and the number of patients is estimated to increase in the coming years.
Recent data suggest that Type 2 Diabetes cases are 95% more prevalent in adults.
Obesity is associated with a pro-inflammatory state marked by chronic elevations of systemic adrenergic activity, dyslipidemia, and hyperglycemia.

How does Obesity contribute to Heart diseases?

Obesity and heart disease

Patients with heart diseases and diabetes are often hyper-tensive. If there is constant metabolic stress to the heart, it leads to the deterioration of the function of the heart.
The relationship between obesity and cardiovascular diseases has been proven by various scientific researches.
Obesity leads directly to heart diseases through atrial enlargement and ventricular enlargement. When the fat increases the body can’t metabolize the food as it should and creates unwanted blockages.
It changes your cholesterol levels and lowers your good high-density lipoprotein (HDL) cholesterol.
Obesity can cause your blood pressure to rise. The pressure required to move around is a lot when one is obese.

The link between diabetes and heart disease

People who are suffering from diabetes are at a higher risk of developing heart diseases. This is due to high blood glucose which damages blood vessels and the nerves responsible for controlling the heart and blood vessels. They could be prone to high blood pressure which further increases the risk of cardiovascular diseases.

Obesity Prevention and control

There are steps that can be taken to reduce the risk of developing heart disease and obesity. Following are some steps one can follow:

  • Keeping your blood glucose,
  • BP, and cholesterol under control
  • Quit smoking
  • Follow a healthy diet plan
  • Engage in regular physical activity
  • Maintain healthy weight
  • Manage stress well
  • Get enough rest

The key to avoiding obesity is to exercise regularly and a healthy diet. In some cases that might not be sufficient. Sometimes it is not about what one eats rather how the excess fat is biologically assimilated. Some metabolic parameters like insulin, glucose, gut microbiome can affect how the food digests and where it goes.

Understanding HRCT and its Importance in the Diagnostic Process

HRCT means High-Resolution Computed Tomography, it is a cross-sectional imaging technique used for prognosis and diagnosis of various health ailments, most common being lung disease. It is usually performed using a conventional CT Scanner, although the imaging parameters may vary.
The technique of CT Scan was originally very different. There were a lot of complications to be able to get a correct reading, for example, earlier the scans were carried out in wide spaces. The image resolution was an issue. Whereas, the modern CT Scans are very efficient and have a great range of different image resolutions.

When should one get an HRCT Scan?

ct scan

A person should go for an HRCT Scan when:

  • Diagnose muscle disorders, fractured bones, and tumors in bones.
  • To identify sinus problems and other pulmonary disorders
  • To locate exact locations of tumors, blood clots, infections, internal bleeding, or internal injuries.
  • To guide procedures like surgeries, biopsies, radiation therapy.
  • To check for bone diseases
  • To examine structures and regular check-ups of various body organs.

Organs that can be scanned through HRCT Scan are:

  • Chest and Pulmonary CT Scan
  • Thoracic and Lumbosacral Spine CT Scan
  • Cranial CT Scan
  • CT Angiographies
  • CT Enterography

The process of HRCT Scan

The process of HRCT scans is harmless and pain-free. The patient has to lie face up or down, depending on the procedure on a sliding scan table, that goes into a hollow circular scanner that has cross-sectional high-resolution X-Rays. Sometimes the patient is injected with Dye to outline the organs for a clearer examination of different body parts.

After the Scan

After the HRCT Scan, the patient can resume his activities as per usual. Fasting patients can take their meals regularly. Reports are prepared by the radiologists, the doctors discuss the reports with the patients about the diagnosis. The reports are usually available after 24 hours of the scan.

How is HRCT beneficial in the Diagnosis process of Pulmonary diseases and COVID-19?

covid 19 in the lungs

There are multiple types of Lung diseases, to be able to diagnose the right one, CT Scans are very useful. HRCT examinations provide valuable information that cannot easily be depicted through medical history and other tests. It ensures accuracy in the prognosis or diagnosis of diseases. It determines the progression of the diseases easily and examines all the major and acute symptoms of worsening of diseases.

Many healthcare institutes prefer using the swab method to identify an infection. Although, according to a research study done by the American Journal of Roentgenology, it was concluded that the CT Scan diagnosis of COVID-19 is fairly accurate with an error rate of 4%. Various people have different preferences regarding diagnosis, HRCT has been a primary tool in the early detection of COVID-19.

What is Internal Medicine And The Role of an Internal Physician

“The good physician treats the disease; the great physician treats the patient who has the disease.” — Sir William Osler, FRS, FRCP

The way a doctor interacts with their patients makes a lot of difference in building longitudinal relationships with people who seek their care. Working alongside the patients, and making their long term wellness a topmost priority goes a long way for a doctor.

Internal Medicine is the branch of medicine that deals with the prevention, diagnosis, and treatment of undifferentiated and multi-system diseases and their processes. It is derived from the German word ‘Innere Medizin’ meaning internal medicine. The doctors were known to be internal medicine doctors or internists because they focused on the “inner” diseases instead of the external manifestations of those diseases. It explores the underlying pathological causes of symptoms and syndromes with the help of laboratory and clinical assessment of bedside patients and emergency wards.

Thomas Sydenham, the father of English Medicine, also developed Nosology-the study of diseases via the clinical approach of diagnosis and management by careful bedside study of the natural history of diseases and their treatment. He took part in the dissection of corpses and scrutinized the internal working of the body for considering the internal mechanics and causes of symptoms.

Following his research, in the 18th Century, there was a high rise in anatomical pathology and laboratory studies combined with the clinical approach with the process of investigations. The research became very prominent with the contribution of pioneer doctors.
Internists or Internal Medicine physicians have multiple roles in a hospital. They tend to various types of illnesses and diagnoses.

Role of Internal Medicine Physicians

internal physician

Due to the complex nature of illnesses and multisystem examinations, internists are highly trained in illnesses and disorders of all parts of the body.
They are expected to address undifferentiated presentations of complexities that cannot be classified within the expertise of single-organ specialists, for example, Dyspnoea, weight loss, change in a conscious state, etc.
They act as primary care physicians wherever required.
Their training extends beyond adult patients, it also involves surgeries, obstetrics, and pediatrics.
They are the first responding doctors for medical emergencies.
Most internists work with medical groups or large hospital system, they see their patients in an office setting.

Medical Procedures

Internal Medicine is focused on the art of diagnosis and treatment with medication, but many subspecialties administer procedural treatment:

  • Allergy and Immunology (immune system)
  • Cardiovascular Disease (heart and vascular system)
  • Advanced Heart Failure and Transplant Cardiology
  • Interventional Cardiology
  • Clinical Cardiac Electrophysiology
  • Endocrinology, Diabetes, and Metabolism (diabetes and other glandular and metabolic disorders)
  • Gastroenterology (gastrointestinal system, liver, and gall bladder)
  • Transplant Hepatology
  • Hematology (blood)
  • Infectious Disease (bacterial, viral, fungal, and parasitic infections)
  • Nephrology (kidneys)
  • Oncology (cancer)
  • Pulmonary Disease (lungs and respiratory system)
  • Rheumatology (joints and musculoskeletal system)

“Cure sometimes, treat often, and comfort always.” — Hippocrates

Education and training of Internal Physicians

The career paths and training of an internal medicine physician vary across the world. Many programs require a pre-med degree prior to applying. In India, a medical school graduate is granted a degree and a license to practice after completing four and a half years of training and success in University examinations. They can then practice as Internal physicians or popularly known as Primary care physicians.

The Medical Council of India’s new competency-based curriculum for medical graduates is a major landmark for medical education in India; it represents a paradigm shift. The Council’s attempt to modernize medical education is laudable and in keeping with recent global trends.

How is Internal medicine different from other medical practices?

A lot of people often confuse family physicians with Internal medicine doctors, the family doctors also treat children occasionally, Internal medicine doctors do not usually tend to children. According to the American College of Physicians, 10-15 percent of patients that family physicians deal with, are children. All internal doctors need inpatient and outpatient settings for their procedures. Internists usually treat adults.
